- This form must be completed by all groups who are accepting delivery at a central location prior to distributing fruit locally within their community.
- If you are only running a Ship-to-Home program, you do not need to complete this form.
- This is not a web-based form, and there is no online submission. You will need to download the form and return via email or fax.
